Item: Health and Vaccination Certificates for Entry-exit Personnel

Legal Basis:
Article 16 of Frontier Health and Quarantine Law of the People’s Republic of China;
Article 99, 100 and 102 of Rules for the Implementation of Frontier Health and Quarantine Law of the People’s Republic of China;
Article 7 of Regulations of the People’s Republic of China on Administration of the Entry and Exit of Foreigners

Eligibility:
1. Foreigners; residents from Hong Kong, Macau, and Taiwan; and overseas Chinese holding permanent residency abroad, who apply for working, studying, or living in China for one year or above;
2. Chinese citizens who are authorized to work, study, visit relatives, or live abroad for more than one year;
3. Chinese citizens who return home after living abroad for more than one year;
4. Chinese employees on international transportations.

Materials Required:

I. Health examination for overseas applicants:
1. Valid identity documents (passport, permits for commuting between the mainland and Hong Kong and Macao, or, Mainland travel permit for Taiwan residents);
2. One 2-inch bareheaded, full-face color photo taken within the latest 6 months;
3．Original copy of a physical examination record issued by the hospital designated by the embassy of the nation, or region, of the applicant. The record content should include the original laboratory test reports, and X-ray films.

Ⅱ. Health examination for applicants in China
1. Valid identity documents (identity card, passport, or, permits for commuting between the mainland and Hong Kong and Macao);
2. Two 2-inch bareheaded, full-face color photos taken within the latest 6 months;
3. The students to study abroad should bring their valid identity cards, two bareheaded full-face color photos taken recently (3 photos needed for those who are going to the former Soviet Union (FSU) countries), the forms that the host university requires, and the originals and copies of vaccination records.

Ⅲ. Prophylactic vaccination for the people entering or exiting China
1. For those who will have both health examinations and prophylactic vaccination, the materials required are the same as for health examination takers;
2. The students to study abroad should bring their valid identity cards, two 2-inch bareheaded, full-face color photos taken within the latest 6 months, and recent physical examination results;
3. The tourists to travel abroad should bring their identity cards, one 2-inch bareheaded, full-face color photo taken within the latest 6 months, and exit and entry certificate (visas or air tickets).

Processing time:
(1) Health Certificate
Generally, health certificate can be obtained on the Next Tuesday after the examination day. For a fast-track service, please submit application before 10:00 am on the examination day, and obtain the certificate that afternoon. If there are abnormal findings in the health examination, additional time will be taken for reexaminations.

(2) Vaccination Certificate
For the applicants who take both health examination and vaccination, the two certificates will be issued the same day; for those who only take prophylactic vaccination, the vaccination certificate will be issued on the vaccination day.
